Please be aware that we are not in-network with any HMO plans at this time.

Our office will verify your benefits and help with information on your coverage including: deductible, copays, coinsurance amounts, and possible financial responsibility. Services are provided with the understanding that you (not an insurance company) are ultimately responsible for the cost of your treatment.

Insurance plans vary and not all services may be covered. It is important to verify with your insurance company that we are in-network, as they sometimes outsource to a third party for mental health services. Please notify us with any updates to your insurance coverage in a timely manner as coverage can change at any time with in-network plans.

If your insurance is out of network and you would still like to utilize our services, we can provide you with a bill to submit for possible out-of-network reimbursement.

Other Important Information:

Payments:  Our practice accepts all major credit cards and cash. We do not accept personal checks. Our office requires a valid credit/debit card to be kept on file when you set up an initial appointment, and throughout your treatment. Payment is expected at the time services are rendered.

Lateness: We understand that there are circumstances beyond your control that may cause you to be late. We will do our best to accommodate situations when able, and if it is not possible to see you, you will be rescheduled for another time. We also understand that sometimes it is just not possible to keep an appointment. In certain circumstances the charge for the appointment time may be waived at our discretion, on a case-by-case basis.

Missing an Appointment:  Please do not miss your scheduled appointment as that time is reserved for you. Appointments must be canceled at least 24 hours in advance. If you cancel your appointment with less than 24 hours’ notice, you will be charged a late cancel fee. Please note your insurance will not cover a missed appointment.
